Review workflow
A coded chart moves from draft to review, then back to the coder or on to approval, and it can only move along
those paths: an aasm state machine refuses anything else, and PaperTrail records who changed what and
when, down to the field. That pairing is how a QA workflow stays honest, because nobody can skip review or
